Understanding Calabar swellings: Assessing subcutaneous loiasis using ultrasound Luzia Veletzky, Jeremy T. Calabar swellings are non-pitting edemas, often on the upper limbs or presenting as unilateral orbital swelling. They are described as itchy or painful, limit mobility, and cause notable morbidity. Although adult Loa loa filariae (macrofilariae) are known to migrate widely through host tissues, their exact pathways and tissue preference (tropism) remain unclear. Whilst the exact mechanisms remain unclear, Calabar swellings are considered to be localized allergic reactions to worm antigens, resulting in angioedema. Leveraging advances in portable ultrasound, this project aimed to characterize Calabar swellings via ultrasound to better understand the pathophysiology behind them. This study was conducted in the Lékoumou department, Republic of Congo, as an ancillary investigation within the MorLo (Morbidity due to Loiasis) project. During a follow-up visit in September 2023, participants and interested villagers were invited to present if they had Calabar swellings. Participants completed a standardized questionnaire and were examined by a physician who also performed ultrasound imaging. Twenty-two individuals presented with visible swellings; 8 (36.4%) were classified as “not typical”, 12 (54.5%) as “typical” Calabar swellings and fourteen (63.6%) had subcutaneous nodules on the forearms or wrists, with 8 (36.4%) having both simultaneous swellings and nodules. Ultrasound videos of typical swellings showed nonspecific angioedema, whereas videos of nodules revealed subcutaneous trans-fascial, possibly fluid filled, retentions extending through fascia and crossing anatomical boundaries with mixed-echogenic content. Within these retentions, hyperechoic thread-like structures were visible, displaying autonomous movement in two videos. Their measured diameters corresponded to L. loa macrofilariae. The ultrasound findings are most consistent with trans-fascial migration of adult L. loa filaria, associated with fascial lesions and fluid retention. The classical Calabar swelling appears closely related to—but not directly over—the underlying lesions. Migration of adult L. loa filaria causes a destructive and disabling process that warrants research and public health responses to ensure appropriate care for affected individuals.

Ultrasound in rheumatic and musculoskeletal diseases: From musculoskeletal to organ involvement Lyubomyra Kun, Irina Gessl, Georgios Filippou, Matteo Ferrito, Roberto Giacomelli, Peter Mandl Joint Bone Spine, 2025 In last two decades, the use of ultrasound has assumed an increasingly important role in the assessment of almost all rheumatic and musculoskeletal diseases. Over the years, much advancement has been made in the sonographic evaluation of articular and periarticular structures, validating definitions of normal sonographic appearance of various types of tissues and structures as well as elementary sonographic lesions and scoring systems to apply in the detection and monitoring of inflammatory and structural lesions. Since extra-articular involvement is very common in rheumatic and musculoskeletal diseases, there is growing interest and evidence on how ultrasound can be applied to the examination of different tissues and organs beyond synovial joints. In this narrative review, we will provide an overview of the available data on the use of ultrasound in the musculoskeletal system (joint, tendon, enthesis, muscle) and extra-articular organs (cardiovascular system, salivary and lacrimal gland, lung, skin and nails), including elementary lesions, scoring systems and their relevance to diagnosis, monitoring, prediction and assessing treatment efficacy in rheumatic and musculoskeletal diseases.
Calcium pyrophosphate crystal deposition: 2025 update to recent epidemiological findings Charlotte Jauffret, Sara K. Tedeschi, Abhishek Abhishek, Augustin Latourte, Georgios Filippou, Tuhina Neogi, Tristan Pascart Current Opinion in Rheumatology, 2025 Purpose of review Our objective is to propose an expert opinion focusing on most important and recent developments in calcium pyrophosphate deposition (CPPD) epidemiology. We highlight recent findings published in the past 18 months and their potential implications for research and patient care. Recent findings We discuss new understanding of CPPD prevalence through advances in imaging modalities, advances in synovial fluid analyses (SFA), updates on disease phenotypes, and potential sources of misdiagnosis of CPPD. We present recent data regarding extra-articular associations of CPPD, particularly cardiovascular events and osteoporotic fractures. We discuss new therapeutic options. We identify barriers to improving research in CPPD, and tools currently available to overcome certain pitfalls. Summary Improved knowledge in the epidemiology of asymptomatic CPPD and symptomatic CPPD disease is crucial to improving recognition of this still underdiagnosed disease, and to understanding patient phenotypes and their outcomes. Future research will require prospective designs to establish the prevalence of CPPD disease phenotypes and to provide more precise data according to each phenotype, both in terms of epidemiological findings and treatment responses, to develop personalized medicine.
Imaging of crystal-induced arthropathies in 2025 Silvia Sirotti, Tristan Pascart, Ralf Thiele, Georgios Filippou Best Practice and Research Clinical Rheumatology, 2025 In recent years, imaging has become an essential tool in the assessment of crystal-induced arthropathies (CIAs), including gout, calcium pyrophosphate deposition disease, and basic calcium phosphate crystal deposition. Advances in imaging have improved diagnosis and disease monitoring, leading to its integration into classification criteria and clinical guidelines. Ultrasound (US), conventional radiography (CR), and dual-energy computed tomography (DECT) each offer unique advantages. US is a widely accessible, cost-effective, and dynamic tool, while DECT provides crystal-specific images, aiding particularly in gout diagnosis. CR, though less sensitive to early crystal deposition, remains valuable for evaluating structural damage and chronic changes. Despite these advances, challenges remain. The specificity and sensitivity of imaging findings need further validation, and the clinical relevance of certain imaging features is debated. This review summarizes recent developments, highlights key strengths, and discusses unresolved issues, emphasizing areas where future research is needed to optimize imaging use in CIAs.

These include difficulties in diagnosis due to diverse clinical presentations, the lack of classification criteria, the absence of standardised diagnostic modalities, underrecognition in clinical care, and, most significantly, the lack of evidence-based treatments. Consequently, CPPD was often regarded as the 'poor cousin' of gout. Fortunately, in recent years, CPPD has garnered increased attention from the rheumatology community. Milestones such as the first American College of Rheumatology/European Alliance of Association for Rheumatology classification criteria, the European Alliance of Association for Rheumatology recommendations for the use of imaging in clinical practice, validated OMERACT ultrasound definitions and scoring system, and the OMERACT core domain sets for CPPD have significantly advanced the field. Yet, unresolved issues regarding CPPD nomenclature hold back advancement in both research and clinical practice. The terminology surrounding CPPD remains inconsistent in the scientific literature, with numerous terms and acronyms used to describe the condition and its manifestations. For example, many 'pseudosyndromes' (eg, pseudogout) and purely radiographic descriptors (eg, chondrocalcinosis) are still commonly used by clinicians and researchers interchangeably, as either the name of the condition or one of its clinical or radiographic manifestations. This lack of standardisation complicates communication among health care professionals, researchers, and between doctors and patients, creating insurmountable barriers to effective care and research advances. In this viewpoint, we highlight the value of standardised terminology, drawing parallels with other rheumatic diseases. We aimed to explore the historical evolution of CPPD nomenclature, assess the impact of previous standardisation efforts, and propose a possible way for a common language.
