Only a few trials explored the role of conventional DMARDs: none of them adequately ruled out subclinical giant cell arteritis (GCA). The aim of this study was to assess whether the precocious administration of methotrexate (MTX) and a short course of glucocorticoids (GCs) could be safe and effective in recently diagnosed PMR. METHODS: Patients with a recent diagnosis of PMR were prospectively enrolled, upon exclusion of GCA, and treated with MTX and gradually tapered prednisone (PDN). As a control group, a historical cohort treated with GC-monotherapy was included. The primary endpoint was PDN-free remission at 24 weeks. Secondary endpoints were sustained remission at 52 weeks, number of relapses, PDN dosage, C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) at 26 and 52 weeks, compared to historical cohort. RESULTS: The prospective and historical cohorts included 41 and 63 patients, respectively. In the MTX-group, at 26 weeks, 66% of patients were in steroid-free remission and a significant reduction of CRP and ESR was observed, while mean daily PDN dosage dropped to 3.42 mg. Twenty-four subjects reached 52-week phase of the study and 83% maintained GC-free remission. A higher number of relapses were reported in the retrospective cohort (12, 27% vs. 34, 43%), in which up to 49 patients were still taking PDN. CONCLUSION: In conclusion, upon a subclinical GCA is excluded, MTX at adequate dosages, is a suitable option for the treatment of PMR, allowing a precocious discontinuation of oral GCs.
Achievement and Maintenance of Remission with Upadacitinib in Patients with Moderate-to-Severe Rheumatoid Arthritis in Italy: 1-Year Data from UPHOLD, a Prospective Real-World Observational Study Caterina Baldi, Eleonora Celletti, Serena Bugatti, Marcello Govoni, Massimiliano Cazzato, Andrea Picchianti Diamanti, Marco Fornaro, Giuliana Guggino, Luca Navarini, Maria Antonietta D’Agostino, Luca Quartuccio, Francesco Ciccia, Lorenzo Dagna, Paolo Stobbione, Massimo Triggiani, Ombretta Viapiana, Annarita Giardina, Gianluca Moroncini, Roberto Caporali, Chiara Bazzani, Enrico Tirri, Claudia Lomater, Sara Di Fino, Francesca Morello, Carlo Selmi Rheumatology and Therapy, 2026 Upadacitinib (UPA) is approved for moderate-to-severe rheumatoid arthritis (RA) based on SELECT trials, but data on real-world effectiveness are limited. UPHOLD is a multicountry study of patients with RA receiving UPA 15 mg. The present interim analysis was based on the Italian cohort performed across 28 centers. Co-primary endpoints were (i) the proportion of patients receiving UPA who achieved DAS28(CRP) remission (< 2.6) at 6 months and (ii) the proportion of patients achieving DAS28(CRP) remission at 6 months who continued to receive UPA and maintained remission (or had no more than a 0.6-point increase in DAS28[CRP]) at 12 months, analyzed by modified non-responder imputation (mNRI) and as observed (AO). Modified full analysis sets (mFAS1 and mFAS2) included patients completing 6 and 12 months, respectively. Safety analysis included reporting of adverse events and treatment-emergent adverse events (TEAEs), as exposure-adjusted event rates (EAERs; events per 100 patient-years [E/100PY]). Among 270 patients, 74 (27.4%) discontinued by 12 months because of lack of efficacy (13.7%) or adverse events (8.1%). In mFAS1 (N = 168), 50.6% (mNRI) and 62% (AO) achieved DAS28(CRP) remission at 6 months. In mFAS2 (N = 55), 80% (mNRI) and 91.7% (AO) maintained DAS28(CRP) remission at 12 months. CDAI and SDAI remission rates at 12 months were 31.3%. Patients on UPA monotherapy at 12 months showed remission rates of 49.5% (DAS28[CRP]), 27.2% (CDAI), and 27.4% (SDAI). Significant improvements in patient-reported pain and physical function were also observed. A total of 278 TEAEs were reported (80.8 E/100PY), including herpes zoster, liver disorders, and serious infections with EAERs of 2.0, 1.5, and 1.2 E/100PY, respectively. UPA 15 mg was observed to effectively treat moderate-to-severe RA in the real-world setting, with ≥ 80% maintaining DAS28(CRP) remission at 12 months, showing a favorable benefit–risk profile. ClinicalTrials.gov identifier, NCT04497597. Rheumatoid arthritis is a chronic autoimmune disease that causes joint pain, swelling, and disability. Although several effective treatments exist, many people with rheumatoid arthritis still struggle to achieve remission and fully control their symptoms. Upadacitinib is a once-daily oral treatment that has shown strong results in clinical trials, but less is known about how well it works in everyday medical practice. This study followed 270 people with moderate-to-severe rheumatoid arthritis who began treatment with upadacitinib across 28 centers in Italy. Rheumatologists monitored their disease activity, pain, physical function, and any side effects over 12 months. After 6 months of treatment, about half of the patients had achieved remission according to a commonly used disease activity measure (DAS28-CRP). After 1 year, more than 80% of those who reached remission were able to maintain it. Other measures of disease control (CDAI and SDAI) also improved, and many patients reported less pain and better ability to perform daily activities. Upadacitinib was seen to be effective both when used alone and when combined with other standard rheumatoid arthritis medications. Side effects were generally consistent with what has been seen in clinical trials, with low rates of serious infections, liver issues, or blood clots, and no major cardiovascular events recorded. Overall, this real-world study shows that upadacitinib is an effective and well-tolerated option for helping people with rheumatoid arthritis achieve and sustain meaningful improvements in symptoms and quality of life.

Wakefield, Paul Emery, Andrea Di Matteo Current Rheumatology Reports, 2025 Purpose of Review This review highlights key ultrasound applications for evaluating extra-articular involvement in rheumatic diseases, including the lungs, vessels, salivary glands, muscles, nerves, skin, and nails. It explores recent advances, emerging areas of assessment, and future research directions. Additionally, the review examines current limitations in the routine use of ultrasound for these purposes and considers the potential of new technologies, such as shear-wave elastography, contrast-enhanced ultrasound, and artificial intelligence, to enhance the early detection and monitoring of extra-articular manifestations in rheumatic diseases. Recent Findings Extra-articular manifestations in patients with rheumatic diseases are crucial for diagnosis, management (including treatment strategies), and prognosis, making accurate assessment essential. Growing evidence supports the role of ultrasound in assessing these manifestations for diagnosis, monitoring, and gaining insights into disease pathogenesis. Recent studies emphasize the significant utility of ultrasound in evaluating extra-articular involvement across various organ systems, including the lungs, vessels, salivary glands, muscles, nerves, skin, and nails. Technological advances, such as shear-wave elastography, contrast-enhanced ultrasound, and artificial intelligence, are expanding the scope and precision of ultrasound applications. Despite its potential, challenges such as operator dependency, lack of standardized protocols, and the need for specialized training hinder its widespread adoption. Summary Ultrasound is a non-invasive, cost-effective, and radiation-free imaging modality with high diagnostic accuracy, making it a valuable tool for assessing extra-articular manifestations in rheumatic diseases. Emerging technologies may further enhance its clinical utility. However, efforts to standardize techniques and improve accessibility are necessary to optimize its integration into routine practice.
Effectiveness and Safety of Recombinant Zoster Vaccine in Rheumatic Diseases: Real-World Evidence from a Single-Centre Italian Cohort Silvia Valentini, Jurgen Sota, Irene Fineschi, Edoardo Conticini, Estrella Garcia-Gonzalez, Emilio D’Ignazio, Marco Bardelli, Stefano Gentileschi, Marta Fabbroni, Francesca Bellisai, Paolo Falsetti, Enrico Selvi, Luca Cantarini, Bruno Frediani, Caterina Baldi Vaccines, 2025 Background: Patients with rheumatic diseases (RMDs) are at increased risk of herpes zoster (HZ), particularly when receiving immunosuppressive treatment. While recombinant zoster vaccine (RZV) has shown high effectiveness in the general population, evidence in rheumatologic patients remain limited due to their exclusion from pivotal trials. Objectives: To evaluate the effectiveness of RZV and to collect additional safety data in a heterogeneous cohort of rheumatologic patients, compared with a control cohort from the general population. Methods: We conducted a retrospective study including 179 adults who received two intramuscular doses of RZV between January 2021 and June 2025. The cohort included 114 patients with RMDs and 65 individuals from the general population. Effectiveness was defined as the ability to prevent HZ reactivation while safety concerns were recorded as any adverse event temporally associated with the vaccination. Results: We observed a statistically significant reduction in terms of VZV relapses following vaccination (p < 0.001). Among patients diagnosed with RMDs, only one case of HZ recurrence was observed 14 weeks after vaccination, with no significant difference compared to general care patients. One patient experienced a disease flare requiring glucocorticosteroids. RZV demonstrated a favourable safety profile, with minor adverse events (fever, injection-site reactions, headache and myalgia) reported in 17.5% of patients after the first dose and 21.5% after the second. No significant association was observed between adverse events and advanced immunosuppressive therapy. Conclusions: RZV displayed an effective and reassuring safety profile in a heterogeneous cohort of patients affected by RMDs, irrespective of the diagnosis and the ongoing therapy. This supports the broader use of RZV as a safe and valuable preventive strategy in patients with RMDs.
Real-world effectiveness and retention rate of upadacitinib in patients with rheumatoid arthritis: results from a multicentre study Caterina Baldi, Stefano Gentileschi, Francesca Li Gobbi, Massimiliano Cazzato, Andrea Delle Sedie, Carla Gaggiano, Emilio D’Ignazio, Gemma Lepri, Chiara De Lorenzo, Carlotta Nannini, Laura Niccoli, Anna Panaccione, Luca Di Cato, Andrea Di Matteo, Andrea Picchianti-Diamanti, Serena Guiducci, Bruno Frediani, Maurizio Benucci Clinical and Experimental Medicine, 2025 This study evaluates upadacitinib (UPA) effectiveness and drug retention rate (DRR) in patients with rheumatoid arthritis (RA). Multicentre prospective observational study. Consecutive patients with RA receiving UPA were evaluated at 0, 3, 6, 12, 18, and 24 months of treatment. Key outcomes included UPA DRR and changes in clinical and serological measures over time. The study included 215 patients (72.6% female sex, mean age 60.1 ± 11.7 years). The DRR of UPA was 91.6% (95% CI 88.0–95.4%) at 6 months, 84.6% (95% CI 79.8–89.7%) at 12 months, 80.3% (95% CI 75.0–86.0%) at 18 months and 80% (95% CI 75.0–86.0%) at 24 months. UPA DRR was similar between monotherapy and methotrexate combination (p = 0.47), and across different treatment lines (p = 0.58). A statistically significant improvement from baseline was observed over 24 months considering erythrocyte sedimentation rate, C-reactive protein (CRP), Health Assessment Questionnaire (HAQ), Disease Activity Score (DAS)28-CRP, Physician’s (Ph) and Patient’s (Pt) Global Assessment (GA), Visual Analogue Scale (VAS) Pain, Simplified and Clinical Disease Activity Index (SDAI and CDAI) (p < 0.00 for all of them). Patients discontinuing UPA were more likely to be male (p = 0.02), with a longer disease duration (p = 0.03), higher baseline values of VAS Pain (p < 0.00), PtGA (p < 0.00), PhGA (p < 0.00), CDAI (p < 0.00), SDAI (p < 0.00) and corticosteroid dosage (p = 0.04). This study confirms UPA effectiveness in managing RA in the real-world practice, demonstrating sustained drug retention and improvements in clinical and laboratory measures over time. Also, UPA could be a valuable option for patients with multi-refractory RA and when monotherapy is preferred.

While pivotal trials and early real-world studies have demonstrated the efficacy of upadacitinib (UPA), data on its impact as assessed by imaging remain limited. Here, we report the 48 weeks outcomes from the full cohort of the UPARAREMUS study. METHODS: UPARAREMUS is a 12-month follow-up, observational study involving RA patients from 9 Italian rheumatology centers initiating UPA. The primary objective was to evaluate the proportion of patients achieving combined clinical and ultrasound (US) remission. Secondary endpoints included the proportion of patients achieving clinical or US remission alone at multiple time points. RESULTS: A total of 115 patients were enrolled. Combined clinical and US remission was achieved in 35% of patients at week 12, increasing to 55% at week 24 and 72% at week 48. Younger age, lower baseline CDAI, and higher ESR were associated with higher remission rates, while baseline corticosteroid use was linked to a 65% lower likelihood of achieving remission. More than 60% of patients showed absence of power Doppler (PD) signal at week 24, rising to over 80% by week 48. The majority of patients in clinical remission also achieved US remission. CONCLUSIONS: After 12 months of UPA treatment, a substantial proportion of RA patients achieved combined clinical and US remission, independent of prior bDMARDs use or monotherapy. These findings highlight UPA's ability to induce and maintain deep disease control in real-world practice.

Oncology, 2025 Background Pancreatic resection with concomitant vascular resection is increasingly practiced with outcomes mainly reported from specialist centers but lacking results from prospective global data. This study aimed to investigate factors associated with short-term outcomes after vascular resections in pancreatic surgery worldwide. Patients and Methods Data were extracted from a prospective, multicenter, international cross-sectional snapshot study in 2021 (pancreasgroup.org) assessing short-term outcomes after pancreatic surgery worldwide (NCT04652271). In the patient cohort of pancreatic surgery with simultaneous vascular resection for various diseases, short-term outcomes were reported and compared with established benchmark values. Factors affecting major complications, mortality, and histopathological resection status were assessed in multivariable logistic regression analyses with interaction testing. Results From a total of 3926 patients undergoing pancreatic surgery, 565 had associated vascular resections, of which 444 had venous resections alone and 121 had arterial resections alone or with concomitant venous resection. Of the 153 (47%) benchmark cases with pancreatoduodenectomy and venous resection, median postoperative morbidity fell within established benchmark criteria. Median 90-day major complication and mortality rates were similar in pancreatic resection with venous, arterial or no vascular resections (45 and 10%, 47 and 6.6%, 42 and 9.6% respectively). Patients undergoing arterial resections that developed a clinically relevant pancreatic fistula faced substantially elevated odds of 90-day mortality (OR 8.8 CI 1.6–48). In pancreatic ductal adenocarcinoma, the R1 rate was 26%, neoadjuvant chemotherapy being protective for both overall and venous-specific margins. Conclusions Vascular pancreatic surgery is performed across diverse healthcare settings worldwide. While perioperative complications were comparable to nonvascular pancreatic resections, the observed 90-day mortality was considerable overall. International collaborative efforts should focus on understanding practice variations and improve accessibility of optimal perioperative care to promote rescue capabilities.
Are There Definite Disease Subsets in Polymyalgia Rheumatica? Suggestions from a Narrative Review Paolo Falsetti, Ciro Manzo, Marco Isetta, Francesco Placido, Alberto Castagna, Maria Natale, Caterina Baldi, Edoardo Conticini, Bruno Frediani Healthcare Switzerland, 2025 Background: Polymyalgia rheumatica (PMR) has a multifaceted onset and course, and making a distinction between true PMR and so-called “polymyalgic syndrome” (that is, similar manifestations caused by different conditions) is far from easy in clinical practice. The existence of subsets within true PMR may further complicate the diagnostic question. Distinguishing PMR subsets from PMR-mimicking conditions does not just carry nomenclature value and speculative significance. Indeed, the correct diagnosis influences treatment, prognosis, epidemiological assessments, and health policies. Objectives: We aimed to (1) ascertain the presence of a definite and peculiar subset/subgroup/cluster of PMR in the scientific literature; (2) describe any possible subset/cluster/subgroup of PMR identified in at least two different studies. Methods: We performed a non-systematic (PRISMA protocol not followed) literature search on Embase and Medline (OVID interface). The following search terms were used: polymyalgia rheumatica, subset, cluster, subgroup, subclinical giant cell arteritis, mimicking conditions, polymyalgia rheumatica-like conditions, immunotherapy, checkpoint inhibitor, acute-phase reactants or acute-phase proteins, vaccination, infection, and calcium pyrophosphate deposition disease or chondrocalcinosis. Each paper’s reference list was scanned for additional publications meeting this study’s aim. Abstracts submitted at conferences or from non-peer-reviewed sources were not included. Results: The initial search yielded 2492 papers, of which 2389 articles were excluded based on title and abstract screening. A total of 103 articles underwent a full-length review, and 84 of them were finally assessed for eligibility. A total of seven large subsets of PMR could be identified: (1) PMR with normal acute-phase reactants; (2) PMR with an infection trigger; (3) PMR with a vaccination trigger; (4) PMR with subclinical giant cell arteritis (GCA); (5) PMR and calcium pyrophosphate deposition disease (CPPD); (6) PMR following immune checkpoint inhibitor (ICI) therapy; (7) PMR with peculiar clinical clusters (based on clinical or statistic clustering methods). Conclusions: PMR with normal baseline acute-phase reactants and PMR with an infection or a vaccination trigger could be categorized as subsets of disease. PMR with subclinical GCA and most cases of PMR/CPPD should be categorized as mimickers. Finally, further studies are required to better categorize some peculiar clinical subsets emerging from cluster analyses, and ICI-induced PMR.
Impact of age and cardiovascular risk factors on the incidence of adverse events in patients with rheumatoid arthritis treated with Janus Kinase inhibitors: data from a real-life multicentric cohort Stefano Gentileschi, Carla Gaggiano, Arianna Damiani, Carmela Coccia, Pamela Bernardini, Massimiliano Cazzato, Francesco D’Alessandro, Giulia Vallifuoco, Riccardo Terribili, Marco Bardelli, Caterina Baldi, Luca Cantarini, Marta Mosca, Bruno Frediani, Serena Guiducci Clinical and Experimental Medicine, 2024 Inhibiting Janus Kinases (JAK) is a crucial therapeutic strategy in rheumatoid arthritis (RA). However, the use of JAK inhibitors has recently raised serious safety concerns. The study aims to evaluate the safety profile of JAKi in patients with RA and identify potential risk factors (RFs) for adverse events (AEs). Data of RA patients treated with JAKi in three Italian centers from January 2017 to December 2022 were retrospectively analyzed. 182 subjects (F:117, 64.3%) underwent 193 treatment courses. 78.6% had at least one RF, including age ≥ 65 years, obesity, smoking habit, hypertension, dyslipidemia, hyperuricemia, diabetes, previous VTE or cancer, and severe mobility impairment. We identified 70 AEs (28/100 patients/year), among which 15 were serious (6/100 patients/year). A high disease activity was associated with AEs occurrence (p = 0.03 for CDAI at T0 and T6; p = 0.04 for SDAI at T0 and T6; p = 0.01 and p = 0.04 for DAS28ESR at T6 and T12, respectively). No significant differences in AEs occurrence were observed after stratification by JAKi molecules (p = 0.44), age groups (p = 0.08) nor presence of RFs (p > 0.05 for all of them). Neither the presence of any RFs, nor the cumulative number of RFs shown by the patient, nor age ≥ 65 did predict AEs occurrence. Although limited by the small sample size and the limited number of cardiovascular events, our data do not support the correlation between cardiovascular RFs—including age—and a higher incidence of AEs during JAKi therapy. The role of uncontrolled disease activity in AEs occurrence should by emphasized.
